自己解決しました。
インストール後に何かプロジェクトをオープンしないとインストールの処理が完了しないようです。
URLﾘﾝｸ(blogs.msdn.com)
の "The first time you launch Visual Studio after downloading and installing the extension, you should see a permissions dialog, shown below."
という記述からプロジェクト開かなくてもVSを起動するだけで良いものと思いこんでしまったのが良くなかった。l
