ちょっとサーバの設定を見直してみます。

header()関数で出力しているものはサーバ上のファイルです。
header ("Content-Disposition: attachment; filename=".$file);
header ("Content-type: application/octet-stream")
readfile ("file/".$file);
exit;
上記を実行する前にテンプレート側のヘッダーを呼び出してしまっているので、
ダウンロードするデータの中にはテンプレート(ヘッダー)+ダウンロードファイルとなってしまっている状態です。
